Student Health Reminders

  • Students who are age 11 or 16 or will be turning 11 or 16 this summer need to get the following state required immunizations to attend public school prior to the beginning of the 2025-2026 school year. 
    • Age 11 needs Tdap and first Meningitis (MCV4) 
    • Age 16 needs second Meningitis (MCV4)
    • Do not get these immunizations until after your child turns 11 and 16 as they will not be valid.
    • If your child has an exemption from immunizations the school will need a copy of your papwerwork. These exemptions must be renewed each school year. 
  • If your child is diabetic, the school nurse will need a current Diabetic Plan of Care on the first day of school.
    • Please read and sign the glucagon permission form as well if your child will have Glucagon at school. 
  • If your child has an Epipen or inhaler the school nurse will need a plan of care from the physician on the first day of school.
    • If you plan on your child self carrying an Epipen and or inhaler please have the doctor sign the Medication Self-Administration Form. Also, you will need to sign the authorization form giving us permission to allow your child to self carry.
  • If your child will be taking any medication at school you will need to fill out the medication form.  All medication must be in the original prescription bottle with the current date. 
  • If your child has a seizure disorder, please have the doctor complete the Seizure Action Plan form.
